Self-Identification Questionnaire Tutorial and Transcript

Welcome to the tutorial for Telefilm Canada’s Self-Identification Questionnaire.

As part of our ongoing commitment to supporting a diversity of talent, we are capturing data as it relates to applications and programs.

This assists in the evaluation, eligibility, and decision-making processes, as well as enhancing Telefilm’s programs and industry initiatives.

Collecting data allows us to gain greater insight into the members of our community and their projects. It also plays a key role in creating a more equitable industry.

To assist you with completing the Self-Identification Questionnaire, we have created this video tutorial, along with other resources available on our website, telefilm dot C A back slash data dash collection.

We appreciate your participation and thank you for your support.

To begin, sign in by clicking the link in the email from the Telefilm address no reply at telefilm dot C A.

For new accounts, the Username and Temporary password can be found in the body of the email.

You will be invited to create a new password. Type the Temporary password in the “Old password” field.

Then, type your new password.

If you received your invitation at the same email address used for your Dialogue account, use those same credentials to sign into the Self-Identification page.

If you receive an “Access Denied” message, click the link in the new email invitation to sign out of the Telefilm account that may already be logged in.

Clearing your browser history can also solve this issue.

On the Self-Identification homepage, click on “Accept Terms of Use” under the Action Items section. If you agree to these terms of use, select “ACCEPT”.

You must complete and submit the Self-Identification Questionnaire in one session for it to be saved.

If the Self-Identification Questionnaire is submitted while incomplete, the mandatory fields will be highlighted in red. 

If there is a question that you would prefer not to answer, the options “I prefer not to answer” or “I do not wish to identify” are available throughout the questionnaire. It is your choice what is included!

Once the questionnaire is complete, please review the Personal Information Collection Statement, and if you agree, click on the box. Then, select “SUBMIT” at the end of the form.

After the Questionnaire is submitted, a confirmation page will appear. Click the link to the Self-Identification homepage.

But wait! You are not done yet – there are still a couple of very important steps to be completed!

Once you return to the Self-Identification homepage, find the Action items section, and select the task titled “Consent to Use”.

This is a very important step to completing your questionnaire.

It is important that you complete the “Consent to Use” step before the expiry date. Otherwise, your entire questionnaire will not be considered as completed.

You will see the answers you submitted for each category. You can review as you provide consent to use for each response.

Review the Personal Information Collection Statement at the bottom of the page.

If you agree with the Personal Information Collection Statement, click on the box, and then select “SUBMIT”.

If you did not complete the task before it expired and would like to have it reactivated to submit, please contact questionnaire at telefilm dot C A.

Please note that, depending on when the request is made, Telefilm may not be able to consider the information in the processing of the application, but it will be able to be used in overall program statistics.

On the Self-Identification homepage, the section “Financing Applications” will show the project number and title that has been attached to your Self-Identification Questionnaire.

To modify your responses for future projects, select “To Modify” in the Current Declaration section.

You’ve now completed the questionnaire.

You may close your browser to end the session.
